Case Manager - PRN / Per Diem
About The Position
Kessler Institute for Rehabilitation is seeking a Case Manager for its inpatient rehabilitation hospital in Marlton, New Jersey. The hospital provides care for individuals facing medical, physical, emotional, and vocational challenges related to brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. The Case Manager assesses patients’ social work needs and provides social work intervention as part of discharge planning. The role’s primary functions include clinical interventions and discharge and care planning management, fiscal management, and payer and referral management. This is a per diem/PRN position requiring weekday availability for 4- or 8-hour shifts. Compensation is $45.00 per hour.
Requirements
- Current licensure in a clinical discipline in accordance with state guidelines or a related discipline, or a bachelor’s or master’s degree in a health or human services discipline.
Nice To Haves
- CCM certification.
- Previous experience in case management and discharge planning.
Responsibilities
- Coordinate health care decisions through a systematic approach to support treatment plans that improve quality and outcomes.
- Coordinate care across the continuum.
- Promote cost-effective care within the allotted time frame.
- Ensure payment of hospital-based services meets patient-related utilization management criteria.
- Implement safe and appropriate discharge plans.
- Assess patients’ social work needs and provide social work intervention as part of discharge planning.
- Manage clinical interventions, discharge and care planning, fiscal matters, and payer and referral processes.
