About The Position
Kessler Institute for Rehabilitation is hiring a Case Manager for its inpatient rehabilitation hospital in Marlton, New Jersey. The hospital provides care for people facing medical, physical, emotional, and vocational challenges related to brain injuries, spinal cord injuries, neurological disorders, orthopedic issues, amputation, and multiple traumas. The Case Manager assesses patients’ social work needs and provides social work intervention as part of discharge planning. The role includes clinical interventions and discharge and care planning management, fiscal management, and payer and referral management. This is a per diem/PRN position with weekday coverage needed for 4- or 8-hour shifts. Compensation is $45.00 per hour.
Requirements
- Current licensure in a clinical discipline according to state guidelines, or in a related discipline; alternatively, a bachelor’s or master’s degree in a health or human services discipline.
Nice To Haves
- CCM certification.
- Previous experience in case management and discharge planning.
Responsibilities
- Assess patients’ social work needs and provide social work intervention as part of discharge planning.
- Coordinate healthcare decisions using a systematic approach to support treatment plans that improve quality and outcomes.
- Coordinate care across the continuum.
- Promote cost-effective care within the allotted time frame.
- Ensure payment for hospital-based services meets patient-related utilization management criteria.
- Implement safe and appropriate discharge plans.
- Perform clinical interventions, discharge and care planning management, fiscal management, and payer and referral management.
