About The Position

NewPath is an innovative behavioral health agency offering various services to youth and families in the Tri-State region. The NewPath Child & Family Solutions Residential Program Case Manager is responsible for assisting clients and families in achieving stated goals and objectives and maximizing independence and daily functioning through interventions and advocacy in our Residential Interventions Program.

Requirements

  • High School diploma and 5 years' experience with SED children OR a Bachelor's degree in relevant field and at least 6 months experience working with SED children.
  • Must be 21 years of age; have a valid driver's license and current automobile insurance.
  • Ability to work flexible hours, including some evenings and weekends.
  • Initiative and ability to work independently.
  • A willingness and comfort level in providing community-based services to a diverse population of clients.
  • Excellent problem-solving skills and the ability to remain calm and professional in crisis situation.
  • Must be able to pass a pre-employment background check & drug screen.
  • A clean criminal background check is required.

Responsibilities

  • Conduct individual, family and group treatment interventions as indicated in the Individualized Service Plan.
  • Consistently monitor the needs of the person served and support systems that are relevant.
  • Comply with all documentation and utilization requirements.
  • Coordinate the necessary evaluations and assessments in order to identify each youth's needs, DLA-20, ISP, PHQ9, Ohio Scales etc.
  • Provide transportation as appropriate and necessary to reach identified goals.
  • Maintain and model appropriate interpersonal relationships.
  • Deliver culturally informed services, and maintain professional objectivity and self-discipline.
  • Adhere to agency risk assessments, policies, protocols, safety questionnaire, on-call procedure, safety planning, and corporate compliance.
  • Ensure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.
  • Participate actively in supervision. Review any problems and concerns with supervisor to ensure effective and efficient program operations.
  • Adjust effectively to agencies implementation of new policies and procedures for improving the quality of care.
