About The Position

The Children's Home of Reading Youth and Family Services - CHOR YFS is seeking a compassionate and dedicated Case Manager for their Residential Program. The role involves providing therapeutic case management services to adolescents and their families within a residential/day treatment team. The Case Manager will act as a liaison between the agency, caregivers, and referral agencies, represent the agency at court hearings and interagency meetings, and coordinate care services, discharge planning, and transportation for residents and their families. Strong communication, organization, time management, and clinical skills are essential for working with adolescents experiencing mental health, emotional, or behavioral challenges. The position also includes providing psychoeducational groups, maintaining accurate documentation, coordinating services and community resources, and monitoring client safety.

Requirements

  • Bachelor’s degree in social work, psychology or other related field.
  • One to three years experience working in a human services field (CASSP system-related setting preferred).
  • At least three (3) written references that demonstrate past educational and/or employment experience.
  • Successful completion of core development orientation program.
  • Must be at least 21 years of age.
  • Valid PA driver’s license (or ability to obtain such license), and a good driving record, with proof of auto insurance coverage.
  • PA Act 33/34 clearances.
  • Pre-Employment physical from a licensed physician (after conditional job offer) verifying person has no communicable diseases or physical conditions that might endanger the health and safety of children or prevent the person from performing essential job responsibilities.

Responsibilities

  • Provide quality therapeutic case management services for adolescents and their families as part of a residential/day treatment team.
  • Act as a liaison and maintain communication between The Children's Home of Reading, the primary caregivers and referral/other agencies.
  • Represent the agency at all court hearings and interagency meetings.
  • Coordinate care services, discharge planning and transportation for residents and their families.
  • Demonstrate strong communication, organization, time management, and clinical skills when working with adolescents experiencing mental health, emotional, or behavioral challenges.
  • Provide psychoeducational groups, case management, supervision, and support to clients and families.
  • Maintain accurate and timely documentation, including treatment plans, progress reviews, and discharge summaries.
  • Coordinate services, appointments, transportation, and community resources for assigned clients.
  • Maintain confidentiality and communicate effectively with families, referring agencies, treatment providers, and other community partners.
  • Participate in treatment planning meetings, court hearings, and interagency meetings as required.
  • Monitor client safety and support treatment goals, behavioral plans, and individualized treatment objectives.
  • Coordinate discharge planning and continuity of care for assigned clients.
