About The Position

The Case Manager provides services to patients and families and serves as a liaison among the treatment team, physician, referral source, community agencies, outside therapists, and third-party reviewers. The role maintains the integrity of assigned patients’ medical records, assists with preparing master treatment plans, initiates discharge planning, contacts discharged patients within established time frames for follow-up, and assists with intake screenings and referrals. The Case Manager also performs other related duties as assigned by the Supervisor.

Requirements

• Bachelor's degree in a social services field.
• Experience with children and adolescents.
• Background check, including FBI/BCI, and drug screen required.
