LOFT Community Services is a unique and dynamic charitable organization that supports people living with complex issues such as mental and physical health challenges, substance use challenges, poverty and homelessness. Serving 19,534 annually and providing 1,839 units of supportive housing last year, LOFT is one of Ontario’s largest mental health service providers of its kind. Since 1984, The 416 Community Support for Women (The 416) has provided a safe and supportive environment for women (cis and transgender) who are experiencing or are at-risk of experiencing homelessness, and/or are facing mental health and substance use challenges. Our team works primarily with women living with unmet needs due to their complex challenges. This program helps reduce barriers to accessing equitable health care services, by working collaboratively with other organizations serving women and systemically advocating for improved quality of life for our clients. The 416 offers a health and wellness program, one-on-one support, and a drop-in program. The 416’s drop-in program offers nutritious meals and scheduled group programming. The groups and activities offer opportunities for learning new skills and building a sense of community through social interactions and discussion groups. Community Support Workers can assist with referrals and connections to shelters and addictions and mental health crisis support. As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
