York Region South Simcoe Case Management provides one to one support to individuals experiencing mental health and/or substance use that impacts their life. Peer Support Workers also offer both social and self-help groups for those affiliated with case management services. We provide peer support groups and individualized case management services to help people achieve their optimal health and well-being in the community. All services are voluntary and free of charge. Our services are offered throughout York Region and South Simcoe. Individual case management meetings are arranged in a mutually agreed upon space including your own home, office site or in the community. Peer support groups are offered in one of LOFT’s office sites. As a Case Manager you will work independently or be part of a team that co-ordinates a support network and links clients living with mental illnesses, addictions and physical health issues. The position attempts to empower the client, thereby increasing their independence, dignity and self-respect in a safe and secure environment. Direct service and support will be provided to clients in areas such as daily living, practical assistance, crisis intervention, linkages to primary care and a range of other services to support the individual’s needs.
