Case Manager
About The Position
Wiley is seeking a dynamic and compassionate Case Manager to support its subacute rehabilitation program and represent the organization within the broader healthcare and senior living community. This highly visible, field-based position focuses on outreach and patient experience. The Case Manager establishes and maintains referral relationships with hospitals, senior living communities, and healthcare professionals, and serves as the main point of contact for patients and families transitioning to Wiley for subacute rehabilitation, supporting them from admission through discharge. The position is based at Wiley Christian Retirement Community in Marlton, New Jersey. The schedule includes weekends, with occasional weekday hours for department coverage.
Requirements
- Current New Jersey nursing license.
- Excellent communication, interpersonal, and customer service skills.
- Warm, empathetic demeanor and ability to support patients and families during transitions of care.
- Self-motivated, able to work independently, and able to manage a field-based schedule.
- Valid driver’s license and reliable transportation.
Nice To Haves
- Background in admissions, case management, or discharge planning.
- Strong understanding of the post-acute care continuum and hospital referral processes.
Responsibilities
- Develop and maintain relationships with hospital discharge planners, case managers, and social workers to identify and coordinate appropriate referrals for subacute rehabilitation at Wiley.
- Visit hospitals, assisted living and independent living communities, physician practices, and senior centers to promote Wiley’s services and generate referrals.
- Participate in community events and networking opportunities to expand awareness of Wiley’s offerings.
- Coordinate the admission process with internal and external partners to ensure timely and seamless transitions.
- Welcome each new subacute rehabilitation patient upon arrival and serve as a consistent point of contact for the patient and/or family throughout the stay.
- Collaborate with admissions, nursing, therapy, and social work teams to support a coordinated, patient-centered care experience.
- Track and report outreach activities, referral trends, and patient feedback to support ongoing quality improvement.
