About The Position

Avenues Recovery Center is a nationwide network of drug and alcohol rehabilitation centers offering detox, residential, PHP, IOP, and outpatient services. The Case Manager advocates for clients, initiates and provides case management throughout treatment, helps resolve barriers to care, and coordinates resources and discharge plans to support clients after treatment.

Requirements

• Excellent communication, multitasking, and time management skills.
• Strong problem-solving and critical thinking skills.
• Warm, empathetic, and respectable personality.

Nice To Haves

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.

Responsibilities

• Advocate for clients by addressing and resolving external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange post-discharge resources as needed, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
