About The Position

Southwest Behavioral and Health Services is seeking a dedicated and empathetic Case Manager to support its mission of delivering compassionate care to enhance lives and improve communities. The role serves as a central support within an interdisciplinary team and works primarily with adults. The Case Manager conducts assessments, helps members access community resources, and coordinates care both within and outside the agency. The position is based with the Lake Havasu City team and pays $17.50–$18.00 per hour.

Requirements

Be at least 22 years of age.
Have a bachelor’s degree in a behavioral health field and one year of relevant behavioral health experience, or an acceptable combination of education and experience totaling two years.
Have a valid Arizona driver’s license and motor vehicle clearance.
Have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive it upon hire.
Complete required drug screening.

Responsibilities

Conduct intake screenings, complete intake paperwork, and make referrals.
Complete annual behavioral health assessments, including service planning and support planning.
Facilitate CFTs and ARTs.
Facilitate groups.
Identify gaps in care for members.
Complete SMI and SED evaluations.
Coordinate care inside and outside the agency and assist members with community resources.
