Case Manager

Friends of Switchpoint Inc•Salt Lake City, UT

About The Position

The Case Manager provides direct resident support and promotes housing stability at the property. The role is centered on helping residents navigate barriers, connect with resources, and sustain their housing, while keeping supportive-service matters within the Case Manager's defined scope and escalating matters appropriately. The Case Manager works alongside property and case-management systems, with the Site Manager managing the property and the Regional Property Manager overseeing the system. The Case Manager supports residents with housing stability, lease understanding and retention, behavioral and financial barriers, inspections, transitions, complaints, emergencies, and connections to community resources and benefits. The role includes developing and maintaining each resident's Housing Stability Plan, coordinating with outside service providers with proper authorization, and representing resident supportive services in cross-department collaboration. The position does not include authority to change rent accounts or approve policy exceptions, nor access to AppFolio. Unresolved or high-risk matters are escalated through the Case Manager Supervisor and Senior Manager-Case Management Compliance. Emergency situations must be addressed immediately under established procedures; escalation must not delay an emergency response.
