About The Position

Avenues Recovery Center is hiring a Case Manager for its newest detox and residential treatment facility in Indianapolis, Indiana. The role supports clients throughout treatment by addressing barriers to care, helping maintain insurance coverage, coordinating resources, and planning for discharge and continuing care. Avenues is a nationwide network of drug and alcohol rehabilitation centers with 21 locations across seven states and programs spanning detox, residential, PHP, IOP, and outpatient services.

Requirements

• Excellent communication, multitasking, and time-management skills.
• Strong problem-solving and critical-thinking skills.
• Warm, empathetic, and respectable personality.

Nice To Haves

• High school diploma or GED preferred.
• One year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.

Responsibilities

• Advocate for clients by addressing and resolving external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management for the duration of treatment.
• Arrange post-discharge resources as needed, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
