Case Manager

Provide quality case management as part of a multidisciplinary treatment team and serve as a communication liaison between families and the treatment team. Develop and implement discharge plans, ensuring arrangements for housing, transportation, and aftercare. Participate in treatment planning meetings and communicate patient and family information, placement details, and aftercare plans. Requirements
- Bachelor's degree in Psychology or a related field.
- Must be organized.
- Ability to work well with adolescent families.
