About The Position

Joseph J. Peters Institute (JJPI), a nationally recognized provider serving children, adolescents, and adult survivors of trauma as well as perpetrators of sexual abuse, seeks a Case Manager in the Safety and Responsibility Program (SRP). The Case Manager will support outpatient activity; facilitate scheduling of intake appointments for adults and youths; complete chart audits; and collect, evaluate, and disseminate performance data. This position reports to the SRP Operations Manager.

  • Good analytical and troubleshooting skills
  • Understanding of human services systems
  • Knowledge of resources available to staff and clients that enhance, coordinate, and complement the services provided at JJPI
  • Working knowledge of Microsoft Office applications, such as PowerPoint and Excel
  • At least two years of experience in a related field, such as public health, public assistance, mental health, social services, case management, etc.

Nice To Haves

  • Working knowledge of SPSS
  • Bilingual in English and Spanish

Responsibilities

  • Facilitate tracking of all youth participants being admitted/maintained in SRP and YSRP.
  • Communicate with stakeholders regarding referral details and collect important participant information.
  • Evaluate and determine payer information for incoming evaluations.
  • Schedule evaluation/intake appointments with the participant or appropriate guardian.
  • Complete intake documentation with all incoming participants in Credible and process post-evaluation documentation.
  • Coordinate case management activity in the Safety and Responsibility Program.
  • Assist the supervisor in providing administrative support to clinical staff.
  • Collect, evaluate, and disseminate court reports and monthly reports to Probation and Court Appointed Representatives.
  • Assist in developing and implementing new, innovative program strategies.
  • Collect, evaluate, and disseminate performance data relative to specified quality improvement programs within outpatient clinics.
  • Generate various reports regarding performance against quality indicators, as appropriate to the individual position.
  • Participate in the planning, development, coordination, and presentation of specific training and educational programs as appropriate to the quality assurance needs of the agency.
  • Assist the Adult Outpatient program as needed.
  • Assist other facility departments as needed.
