About The Position

The Case Manager provides a variety of in-home services and referrals to homeless families residing in transitional or permanent shelter programs. The role helps strengthen family units and prepare families for independent living while maintaining the privacy and confidentiality of guests and clients.

Requirements

• Bachelor's degree or equivalent experience.
• Effective interpersonal, verbal, and written communication skills.
• Ability to identify client needs and determine when to involve outside agencies.
• Ability to maintain the privacy and confidentiality of guests and clients.

Responsibilities

• Provide consistent care to develop trusting relationships with family members.
• Work with program staff to coordinate services and referrals for homeless families.
• Educate parents about childcare, personal growth and development, and educational or vocational goals.
• Work with housing workers and families to develop budget plans and support families in following their budget goals.
• Encourage families to seek and continue good health care and help them follow recommendations that support good health.
• Maintain professional relationships with clients and conduct business-related activities according to program policies and standards.
• Participate in developing individual service plans, refer clients to needed community resources, and record necessary information.
• Attend seminars or formalized courses and maintain knowledge of community resources.
