The Case Manager coordinates integrated case management for adults, youth, and young adults, including participants referred through PREVAIL, a voluntary community-based deflection program intended to reduce criminal justice involvement by addressing behavioral health, substance use, housing, benefits, and other social service needs. The role uses trauma-informed, harm-reduction, and person-centered approaches to engage participants, assess needs, plan individualized goals, advocate, coordinate services, and link people to formal and informal supports that promote wellness, safety, stability, and community connection. The position provides intensive case management, outreach, referral management, service coordination, documentation, and data reporting. It works closely with law enforcement and criminal justice partners, schools, behavioral health providers, housing and social service agencies, medical teams, and internal treatment teams to support participant engagement, program enrollment, progress monitoring, and connection to wraparound services. The role also supports participants with daily living needs, benefits, transportation, housing stability, appointments, education or employment connections, and stabilization resources within program guidelines. This is a full-time position based in Fort Collins, Colorado, with a Monday–Friday schedule of 8:00 a.m.–4:30 p.m. Regular local travel to client homes, partner locations, appointments, events, and SummitStone facilities is required; the position may transport clients to authorized appointments and community activities. The listed pay range is $24.04–$28.85 per hour, based on relevant years of experience.
