About The Position

Community Care is hiring a Case Manager to provide community case management for elderly individuals and adults with physical and intellectual disabilities, helping them live as independently as possible in the community. The role manages an assigned caseload in collaboration with an RN Case Manager and includes member assessments, care planning, care coordination, service monitoring, and documentation. The position is full-time, based in Dane County, and follows a weekday schedule of Monday through Friday, 8:00 AM to 4:30 PM. Regular local travel is required.

Requirements

• Bachelor’s degree in Human Services or a closely related area; a minimum of three years’ experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel requiring use of a personal vehicle, a valid driver’s license, and proof of required insurance coverage.

Nice To Haves

• One year of experience with older adults or persons with physical and/or intellectual disabilities in a social service capacity is preferred.

Responsibilities

• Manage an assigned caseload of program members in collaboration with an RN Case Manager.
• Complete comprehensive member assessments, member-centered care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in accordance with established standards.
