About The Position

The Case Manager (CM) is an important member of the treatment team. The CM participates in developing the patient’s plan of care and facilitates efficient throughput, conducts utilization review, promotes appropriate resource utilization, identifies needs, and establishes safe and appropriate discharge plans. These functions are performed throughout the care continuum through sound assessment and collaboration with the patient and caregivers, Attending Physician, Utilization Review Coordinator, and other treatment-team members.

The schedule is 8:00 AM to 4:30 PM, Monday through Friday, with weekends indicated. The role serves patients of all age groups.

Requirements

• Associate Degree in Nursing, or a Master’s degree in Social Work or a related field.
• For new-hire RNs effective June 14, 2017, obtain a BSN from an accredited school of nursing within three years of hire or transfer into the position.
• Licensed to practice nursing in Florida for those meeting the ASN/BSN education requirement; a Master’s degree in Social Work or a related field is accepted in lieu of RN licensure.
• Excellent verbal and written communication skills.
• Excellent organizational skills.
• Strong personal computer skills, including email, Word, and Excel.
• Good typing skills (20 wpm).
• 1–2 years of experience in discharge planning, case management, managed care, or Registered Nurse experience in a medical setting.

Nice To Haves

• Preferred education: BSN, MSW, or Case Management Certification.
• Discharge planning and case management experience in a hospital setting.

Responsibilities
