Seeking a professional, experienced case manager familiar with shelters and housing. The candidate will work with vulnerable, indigent male and female adults, develop and coordinate service plans for a full caseload assigned, including resident and drop-in populations. This role involves utilizing day-to-day working relationships with internal programs, outside agencies, and resources relevant to the client population. Responsibilities include making appropriate referrals and following up on them, participating in weekly one-on-one supervision and case conferences, organizing special didactic activities for clients, following up on special problems of individual clients, maintaining and regularly updating client records, and submitting program reports as required. The role also involves engaging and referring to local treatment providers serving chronically homeless individuals suffering from disorders of mental illness and substance abuse. Utilizing trust-building, motivational interviewing, and integrated treatment methodologies, the case manager will monitor progress and make referrals to additional support services focused on helping clients achieve housing stability.
