About The Position

The Pediatrics Inpatient Case Manager at Comer Children's Hospital is part of UChicago Medicine’s Care Coordination team and partners with the clinical team to support high-quality care and seamless continuation of care. The Case Manager is a member of the leadership team responsible for developing and maintaining patient care standards. The role coordinates and oversees patient care primarily during the inpatient phase of treatment, collaborating with the care team and multidisciplinary team according to patient needs and appropriate use of resources.

Requirements

Current Illinois Registered Nurse license.
At least 3 years of nursing experience.
Required qualifications list a bachelor’s degree in nursing from an NLN-accredited school of nursing.
Required education section lists an Associate Degree in Nursing; associate degree nurses must enroll in a BSN program within 6 to 9 months of hire and make progress toward completing the BSN within 30 months.

Responsibilities

Plan, monitor, and coordinate patient care using the plan of care or clinical pathway.
Identify patient and family needs for consultation with multidisciplinary specialists and ensure those needs are met.
Plan and conduct or participate in patient care conferences.
Assess patient discharge needs.
Ensure all discharge-related functions are completed on time.
Consult with staff nurses in evaluating current patient status and the plan of care.
Ensure comprehensive information needed to document the patient's hospital course is available to all care team members.
Document in the patient record as appropriate.
Perform other duties as assigned.
