About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role reviews inpatient admission records to support appropriate utilization of hospital resources and determine whether admission is at the appropriate level of care. The Case Manager assesses discharge needs and coordinates an appropriate discharge plan, working collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The role also maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

State licensure in the state where the hospital resides is preferred.
Experience in an acute, IRF, or LTACH setting is preferred.
Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
