About The Position

The Case Manager coordinates the interdisciplinary care plan, discharge planning, and communication about patient progress in an inpatient rehabilitation setting. The role serves as a liaison among patients, caregivers, and the healthcare team to support timely, effective care transitions. The position is PRN and works on an as-needed basis at UC Davis Rehabilitation Hospital, a 52-bed inpatient acute rehabilitation hospital operated jointly with Lifepoint Health and UC Davis Health.

Requirements

• Hold a California license as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.

Nice To Haves

• Certification in Case Management or Rehabilitation Nursing, such as CCM, ACM, RN-BC, or ARN.
• At least 2 years of experience in social work or case management in an inpatient setting.
• Experience in an acute rehabilitation hospital is strongly preferred.

Responsibilities

• Collaborate with interdisciplinary team members to identify barriers to care or discharge and develop appropriate solutions.
• Document patient care activities in accordance with workflow timelines, including completing the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedule family conferences and communicate patient progress and discharge planning to caregivers after team conferences and as needed.
• Coordinate weekly interdisciplinary team conferences to monitor treatment goals and outcomes.
• Review assigned Case Mix Groups (CMGs), assist the team in identifying actively treated comorbid conditions, and communicate findings to the Health Information Management (HIM) team.
