About The Position

Avenues Recovery Center is seeking a Case Manager to support clients receiving addiction treatment. The Case Manager advocates for clients, addresses barriers to treatment, and initiates case management when clients are admitted. The role includes monitoring clients’ insurance and accounts throughout treatment, providing ongoing case management, and coordinating services and resources needed after discharge. The Case Manager also develops comprehensive discharge plans that address continuing care, housing, and sober support.

Requirements

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.
• Excellent communication, multitasking, and time-management skills.
• Strong problem-solving and critical-thinking skills.
• Warm, empathetic, and respectful personality.

Responsibilities

• Advocate for clients and address and resolve external barriers to treatment.
• Initiate case management at intake and address clients’ pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management throughout treatment.
• Arrange post-discharge resources as needed, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
