About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population. The role includes reviewing inpatient admission records to ensure appropriate utilization of hospital resources and determine whether admissions are at the appropriate level of care. The Case Manager identifies discharge needs and coordinates appropriate discharge plans, collaborates with the interdisciplinary team to support desired financial and quality outcomes, and maintains and enhances payor relationships.

Requirements

Current BLS certification.
Three to five years of inpatient experience.

Nice To Haves

State licensure in the state where the hospital resides is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.
Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
