About The Position
PAM Health is seeking a Clinical Navigator Evaluator to complete patient preadmission assessments and assessment updates. The role works collaboratively with business development team members, including the Admissions Manager and admissions staff, as well as nursing and other internal and external staff, to facilitate referral conversion. The position works under the direction of the Director of Strategic Initiatives and Integration. PAM Health provides specialty healthcare services through more than 80 locations, wound clinics, and outpatient physical therapy locations in 17 states. Its mission is to serve people with compassionate, expert care and support recovery through education and research.
Requirements
- Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
- At least two years of experience in the designated field of licensure.
Nice To Haves
- A degree in a health-related field from an accredited college or university is preferred.
- Prior marketing and/or rehabilitation or LTACH experience is preferred.
