About The Position

The Case Manager for the ESH Program provides comprehensive, on-site case management services to families and single adults experiencing housing instability. This role is responsible for developing individualized case plans focused on stability, resource connection, and permanent housing placement. The Case Manager works collaboratively with participants to identify barriers to housing, establish realistic goals, and coordinate supportive services that promote long-term self-sufficiency. This is an on-site position requiring consistent in-person engagement to ensure effective service delivery, relationship-building, and accountability.

Requirements

  • A Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred.
  • Minimum 2 years of experience in case management, housing navigation, or homeless services.
  • Experience working with families and individuals experiencing housing instability.
  • Knowledge of Housing First principles and local housing resources.
  • Strong written and verbal communication skills.
  • A valid CA driver's license, proof of insurance, and be approved by RP insurance as a driver in order to drive agency vehicles.
  • Ability to complete all required background checks. Additional requirements may be deemed necessary, including fingerprinting (Live Scan), completion of all Community Care Licensing (CCL) documents, a suitability process through ORR and ability to meet CCL licensing requirements.

Responsibilities

  • Conduct intake assessments and develop individualized, goal-oriented case plans.
  • Collaborate with participants to identify housing barriers and stability needs.
  • Facilitate housing navigation, including rental applications, landlord engagement, and housing search support.
  • Support participants in obtaining necessary documentation (ID, income verification, benefits).
  • Provide referrals and warm handoffs to community-based services (employment, healthcare, behavioral health, childcare, etc.).
  • Monitor participant progress toward housing and stability goals.
  • Maintain accurate, timely, and thorough case documentation in agency systems.
  • Ensure compliance with City of Alameda ESH contract requirements.
  • Complete required reports, case notes, and service tracking documentation.
  • Adhere to confidentiality standards and mandated reporting laws.
  • Participate in audits and monitoring visits as required.
  • Maintain consistent on-site presence during scheduled hours.
  • Conduct regular in-person check-ins with participants.
  • Support crisis intervention and de-escalation when needed.
  • Collaborate with on-site staff to ensure coordinated care.
  • Promote a respectful, safe, and supportive program environment.
  • Coordinate with City of Alameda representatives as needed.
  • Participate in case conferencing and multidisciplinary meetings.
  • Build and maintain relationships with local service providers.
  • Support cross-department collaboration within the agency.
  • Complete functional timesheets by assigned due dates in compliance with time allocations.
  • Actively participate in reflective supervision sessions.
  • Create and maintain documentation of processes and procedures related to job duties.
  • Other duties as assigned.
