About The Position

Real House Inc. is seeking a compassionate and organized Case Manager to support clients experiencing mental health, substance use, and co-occurring disorders. The Case Manager will maintain a caseload of approximately 15–20 clients, conduct intake interviews, develop service connections, maintain client records, and coordinate care with clinical staff, healthcare providers, legal representatives, probation and parole departments, and community agencies. Additional responsibilities include helping clients access housing, healthcare, financial assistance, identification documents, education, and other community resources; providing crisis intervention; facilitating rehabilitation and wellness workshops; completing required documentation and monthly reports; and contributing to a team that provides 24/7 client support.

Requirements

High school diploma
Strong communication skills
Strong computer skills
Valid driver's license
Ability to work effectively with diverse populations
Ability to lift up to 35 pounds
Pass a background check
Individuals in recovery must have five to six years of sustained recovery.
