About The Position

The Case Manager RN assists with the collaboration, research, development, implementation, revision, and reporting of the case management program. Working with appropriate members of the health care team, the role coordinates patient care through the development and use of standards of patient care and nursing practice. The Case Manager ensures that specific patient outcomes are achieved and that variances from established and accepted parameters are evaluated and addressed as necessary.

Requirements

• Minimum of two years of clinical experience.
• Working knowledge of health care reimbursement issues.
• Effective oral and written communication skills.
• Self-directed, self-starter able to implement new programs.
• Current California Board of Nursing Registered Nurse License.
• Current American Heart Association BCLS certificate.
• Current California Driver’s License, clear DMV record, and current California auto insurance.
• Computer skills and knowledge of Microsoft Word, Windows, and Excel.
• Electronic Medical Record skills.

Nice To Haves

• Utilization Management experience.
• Understanding of systems and change process.
• CPHQ Certification.

Responsibilities
