About The Position
The Case Manager coordinates interdisciplinary care for a designated patient population. The role reviews inpatient admission records to assess appropriate utilization of hospital resources and determine whether admissions meet the appropriate level of care. The Case Manager identifies discharge needs and coordinates suitable discharge plans, works with the interdisciplinary team to support desired financial and quality outcomes, and maintains and enhances payor relationships. This is a PRN day-shift position at Round Rock Rehab.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience.
- Professional discipline may be occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
Nice To Haves
- Licensure in the state where the hospital resides is preferred.
- Inpatient experience in an acute, IRF, or LTACH setting is preferred.
