The Case Manager may be assigned to either the Employment program or the Housing program. In the Employment program, the Case Manager conducts outreach to eligible homeless and unemployed Texas residents and connects them with support services, benefits information and assistance, emergency shelter or other housing referrals, job training, job placement, and follow-up services. The role includes meeting measurable client service goals and coordinating with organizational programs and community resource centers to avoid duplicating services and maximize client support. Case Managers are expected to treat clients with sensitivity, respect, and professional care, maintain detailed case notes, submit required reports on time, and perform other tasks assigned by supervisors. In the Housing program, the Case Manager develops a case management plan with clients to assess needs and barriers and provide housing placement, temporary financial assistance, emergency housing when needed, and other support services. The goal is to stabilize at-risk veteran families so they can gain or retain permanent housing and strengthen self-sufficiency. The role also involves maintaining client files and correspondence, processing support-service requests and expense verification, submitting client information in the management information system, and working toward assigned client goals. Case Managers must safeguard confidential information and demonstrate responsiveness, adaptability, teamwork, a solution-oriented approach, and sound work standards. Work includes office duties, client and team meetings, local travel and occasional out-of-area travel, and recruiting clients at job fairs, community events, shelters, streets, and encampments. The employee may work in varied office and residential settings and encounter varying weather conditions or situations requiring de-escalation.
