About The Position

AMIkids is seeking a Case Manager to oversee service planning and transitions for youth entering and exiting the program. The Case Manager develops individualized plans for each youth and monitors their effectiveness. The role includes conducting assessments, coordinating treatment planning, delivering evidence-based treatment, and representing the program in court and other proceedings.

Requirements

• Bachelor’s degree in psychology, social work, or a related field.
• At least 2 years of experience working with youth.

Responsibilities

• Complete intake needs assessments, reassessments, and transition planning.
• Evaluate, develop, and document an individual service or care plan for each youth based on the needs assessment, and complete required updates.
• Participate actively in the Multidisciplinary Treatment Team (MDT).
• Deliver evidence-based treatment models while adhering to fidelity requirements and documentation standards.
• Attend and represent the program at court hearings, meetings, and youths’ legal proceedings.
