About The Position

Avenues Recovery Center is hiring Case Managers for its facility in Baton Rouge, Louisiana. The Case Manager advocates for clients, addresses barriers to treatment, initiates case management at intake, monitors insurance coverage, and provides support throughout treatment. The role also includes arranging post-discharge resources and coordinating comprehensive discharge plans.

Requirements

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.
• Excellent communication, multitasking, and time-management skills.
• Strong problem-solving and critical-thinking skills.
• Warm, empathetic, and respectable personality.

Nice To Haves

• High school diploma or GED.
• At least one year of experience in the treatment industry.

Responsibilities

• Advocate for clients by addressing and resolving external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management for the duration of treatment.
• Arrange post-discharge resources such as housing, financial aid, legal aid, and vocational training as needed.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
