About The Position

The Case Manager is responsible for implementing the Case Management System and working with the Interdisciplinary Team to help ensure that people supported are reaching their goals. The role provides advocacy and support to individuals served in ABI, CBDS, and/or Day Habilitation programs. The Case Manager also ensures that documentation is completed and filed in accordance with Opportunity Works policy and applicable licensing and accreditation standards.

Requirements

• At least a high school diploma or equivalent.
• Experience with case management and case reviews in a relevant health care setting.
• Excellent organizational skills.
• Excellent verbal and written communication skills.

Nice To Haves

• Bachelor’s degree preferred.
• Valid driver’s license and ability to drive a van preferred.
• Crisis intervention training, such as PABC, is favorable.
• CPR, First Aid, and MAP certificates preferred.
