About The Position

DeCoach Recovery Centre provides comprehensive, compassionate, evidence-based care for people with substance use disorders, tailoring treatment to each client’s needs and focusing on long-term recovery and wellness. The Case Manager supports adults with substance use and mental health disorders, primarily in community settings, connecting them with resources, practical skills, and support to help them achieve their recovery goals. The role works under clinical supervision as part of a collaborative treatment team and may use flexible hours to meet clients’ needs.

The Case Manager provides services in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and care coordination. The role also includes advocating for clients, helping them develop daily living skills, collaborating with treatment and administrative teams, recommending service improvements, and maintaining documentation in accordance with company procedures and policies.

Requirements

• High school diploma or equivalent.
• At least 1 year of experience in behavioral health or a related field.
• Working knowledge of behavioral health issues and current therapeutic methods.
• Excellent communication skills.
• Active Ohio driver’s license and a clean driving record for the past 3 years.

Responsibilities

• Provide primarily community-based case management to adults with substance use and mental health disorders, using a company vehicle.
• Connect clients with services and resources, advocate on their behalf, and help them build daily living skills.
• Deliver services in each client’s Individual Treatment Plan, including individual and group case management, community activities, facility visits, vocational training, and coordination of care.
• Collaborate with the Treatment Team to ensure each client’s plan fits their needs.
• Partner with administrative staff and team managers to review services, assess needs, and recommend improvements.
• Follow documentation procedures and stay current on DeCoach policies and procedures.
