About The Position

The Case Manager at Tatum Psychology Employment Group is responsible for overseeing documentation of patient’s intake, progress, and discharge, as well as liking them with appropriate social, occupational, and educational resources as needed. The Case Manager works well with the treatment team to plan and coordinate the client’s progress through the continuum of care. This role involves patient admission and orientation, assessment and treatment planning, case management and patient monitoring, communication and interdisciplinary collaboration, discharge planning and post-care coordination, and staff development and training. The position also includes additional functions such as performing other assigned duties, following policies and procedures, collaborating with partners, representing the organization, occasional travel, maintaining regular attendance and punctuality, displaying creativity, and expanding job knowledge.

Requirements

Bachelor's Degree in Psychology, Social Work, or related field.
Two (1) years’ experience in a behavioral health care setting.
Strong leadership, communication, and organizational skills.
Ability to work independently and as part of a team.
Experience working with culturally diverse individuals and communities or have otherwise demonstrated a commitment to strengthening engagement of a diverse community and skill in communicating with diverse populations.
Familiarity with equity and/or diversity initiatives within an organization.
Written and oral communication skills sufficient to perform essential functions.
Demonstrated ability to make decisions independently and use initiative to accomplish complex assignments with general instruction and guidance.
Demonstrated ability to exercise judgment and integrity on the job with confidentiality, tact, and diplomacy.
Demonstrated ability to organize time and other resources to perform multiple tasks.
Demonstrated ability to complete work accurately and in a timely manner with attention to detail.

Proficiency in word processing and database and/or spreadsheet applications.
Physical and mental attributes sufficient to perform essential functions.
Valid Driver's License / Clean Driving Record
Ability to pass a Department of Justice (DOJ) Background Clearance

Responsibilities

Admits new patients by completing the intake process and new patient orientation.
Evaluates patients’ needs for medical, psycho-social, and psychiatric treatment.
Monitors cases by verifying patients’ attendance, observing, and evaluating progress in treatment; advocating for needed services; obtaining additional resources; providing personal support.
Maintain patients’ records by reviewing case notes and documenting progress.
Communicates patients’ progress by participating in weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
Prepares patients’ discharge by creating discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
Prepares reports by collecting, analyzing, and summarizing treatment and results data and trends; compiling statistics.
Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
Improves staff competence by providing educational resources; balancing work requirements with learning opportunities; evaluating the application of learning to changes in treatment results.
Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.
Performs other duties as may be assigned.
