About The Position

The Case Manager is responsible for interdisciplinary coordination of care for a designated patient population at a new inpatient rehabilitation hospital. The role reviews inpatient admission records to support appropriate utilization of hospital resources and determine whether admissions are at the appropriate level of care. The Case Manager identifies discharge needs and coordinates appropriate discharge plans, collaborates with the interdisciplinary team to achieve desired financial and quality outcomes, and maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

Licensure in the state where the hospital resides is preferred.
Experience in an acute, inpatient rehabilitation facility (IRF), or long-term acute care hospital (LTACH) setting is preferred.
