About The Position

The Case Manager coordinates care across disciplines for a designated patient population. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs and coordinates suitable discharge plans, works with the interdisciplinary team to achieve desired financial and quality outcomes, and maintains and enhances payor relationships.

Requirements

Current BLS certification is required.
Three to five years of inpatient experience.

Nice To Haves

State licensure where the hospital is located is preferred.
Inpatient experience in an acute, IRF, or LTACH setting is preferred.

Responsibilities

Coordinate interdisciplinary care for a designated patient population.
Review inpatient admission records to ensure appropriate utilization of hospital resources and determine whether admission is at the appropriate level of care.
Assess and identify discharge needs and coordinate an appropriate discharge plan.
Collaborate with the interdisciplinary team to facilitate desired financial and quality outcomes.
Maintain and enhance payor relationships.
