Case Manager - FT | South Point Rehab
About The Position
The Case Manager is responsible for coordinating care across an interdisciplinary team for a designated patient population at a new inpatient rehabilitation hospital. The role reviews inpatient admission records to support appropriate use of hospital resources and determine whether patients are admitted at the appropriate level of care. The Case Manager assesses discharge needs, coordinates appropriate discharge plans, and works collaboratively with the interdisciplinary team to achieve desired financial and quality outcomes. The position also maintains and enhances relationships with payors.
Requirements
- Current BLS certification is required.
- Three to five years of inpatient experience.
Nice To Haves
- State licensure in the state where the hospital resides is preferred.
- Experience in an acute, inpatient rehabilitation facility (IRF), or long-term acute care hospital (LTACH) setting is preferred.
- Professional discipline may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
