About The Position

DeCoach Recovery Centre provides comprehensive, compassionate care to people struggling with substance use disorders, using evidence-based treatments and therapies tailored to each client’s needs and focused on long-term recovery and wellness. The Case Manager provides direct client services addressing individual needs, including linkage to resources, development of activities of daily living skills, advocacy, and support for personal recovery goals. The role includes visiting clients at local nursing homes, taking vitals, administering drug screens, and arranging telehealth visits with providers. Services are mostly community-based, and the position may use flexible hours to best serve clients. The role requires a properly insured company vehicle and is performed under clinical supervision.

Requirements

• Graduate from an accredited college or university, or have a high school education.
• At least one year of experience in a behavioral health or related field.
• Active Ohio driver’s license.
• Excellent communication skills.
• Properly insured company vehicle is required for mostly community-based services.

Responsibilities

• Provide case management under clinical supervision to adults with substance use and mental health disorders.
• Link clients to resources and support development of activities of daily living skills, advocacy, and personal recovery goals.
• Visit clients at local nursing homes, take vital signs, administer drug screens, and arrange telehealth visits with providers.
• Provide individual and group case management, community activities, facility visits, vocational training, and coordination of care as specified by the Treatment Team and the client’s Individual Treatment Plan.
• Use a properly insured company vehicle to provide mostly community-based services and meet clients where they are.
• Use excellent communication skills to facilitate client care.
• Comply with all new and current DeCoach Rehabilitation Centre policies and procedures.
• Work with administrative and team managers to improve service quality through ongoing studies of existing services and needs assessments for improvement and revisions.
• Maintain direct knowledge of behavioral health issues, correct documentation procedures, and current therapeutic methods.
