About The Position

Care Lync is hiring a full-time Case Manager to provide activities that meet the needs of clients assigned to the consolidated care team. The role focuses on understanding participants’ disabilities and associated needs, helping them achieve their goals, and developing individualized plans. Plans are modified annually or when a significant medical or social change occurs. The Case Manager coordinates with individuals and entities to support seamless coordination among physical, behavioral, and support services. The position also involves developing and maintaining professional relationships with participants and their families. Care Lync is a social services agency providing support coordination and case management to people with physical disabilities enrolled in Medicaid waiver programs across Nevada.

Requirements

BS/BA degree in related coursework.
No prior experience required.
Experience with Microsoft Office and state-maintained databases.
Ability to maintain proficiency in company-sponsored training and certifications.
Ability to maintain updated state-related clearances.
Ability to successfully complete and maintain training courses required or amended by program regulations.
Valid driver’s license, personal vehicle insurance coverage for liability, and current vehicle registration; copies must be maintained on file with Care Lync.

Responsibilities
