About The Position

Clinton Counseling Center is a private, nonprofit outpatient substance abuse and mental health clinic located in Mt. Clemens, Michigan. The Case Manager/Supports Coordinator provides support to individuals receiving services through the outpatient clinic, connecting them with community resources and advocating on their behalf. The position is full-time and contractual, with benefits, flexible hours, and some local travel in the community. Appropriate supervision is provided by a fully licensed LPC or LMSW.

Requirements

Bachelor's degree in Social Work, Counseling, or Psychology.
CADC/CAADC certification, or an MCBAP Development Plan at the time of hire.
Valid driver's license and own vehicle for use.
Ability to pass a criminal background check.

Nice To Haves

Master's degree preferred; credentials listed include L/LMSW, L/LPC, or T/LLP.
Experience preferred but not mandatory.

Responsibilities

Assess and reassess clients assigned to a caseload.
Plan and/or facilitate planning using person-centered principles.
Develop an Individual Plan of Service (IPOS) using the person-centered planning process.
Review and update the IPOS at least annually or whenever the client wants to review and update it.
Link clients to, coordinate with, follow up with, and advocate with other mental health services and community services or supports.
Broker with providers of services and supports.
Assist clients with access to entitlements and/or legal representation.
Coordinate with other care providers.
Document clinical information completely and in a timely manner according to accepted standards.
Participate in case reviews as needed.
Monitor services provided to the client.
Provide crisis intervention.
Provide discharge planning.
Conduct outreach and networking with other service providers as needed.
