At CORE, Case Managers provide individualized support that helps clients build stability, develop skills, and work toward their personal goals. Through advocacy, resource coordination, and evidence-based practices, you'll help clients navigate challenges, access services, and make meaningful progress on their recovery journey while fostering independence and self-sufficiency. This title is used for Case Managers who are working toward certification. Case Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting their individualized goals. Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
