About The Position

Valley's CORE I program provides safe, evidence-based residential treatment for adults navigating mental health challenges, substance use disorders, trauma, and other complex life barriers. As a Case Manager, you'll play a vital role in helping clients achieve meaningful progress by providing rehabilitative services, coordinating care, supporting individualized goals, and building trusted relationships that foster lasting recovery. Requirements

  • High School diploma or equivalent
  • Case Manager certification (within 120 days)
  • CPR certification (post hire if needed)
  • Valley de-escalation certification (post hire)
  • Minimum age of 21 and a current driver’s license (for driving positions)

Nice To Haves

  • Previous social services or behavioral health experience
  • Bachelor’s degree in Social Services, Psychology, or related field

Responsibilities

  • Provides psychosocial rehabilitative services as described by the Medicaid manual
  • Supports clients in following individualized care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Transports clients to appointments as necessary
