The APS Case Manager is responsible for delivering short‑term, client‑centered care coordination and supportive services to APS clients with complex medical, psychosocial, functional, and housing‑related needs. The position emphasizes risk mitigation, service coordination, and housing stabilization, while respecting client autonomy, cultural preferences, and legal rights. The APS Case Manager works collaboratively with APS Investigators, multidisciplinary teams, community partners, and service providers to promote safety, reduce risk, and support clients’ ability to remain safely housed in the least restrictive environment.
