To help share our mission and expand our community impact, Vitra Health is seeking a Non-Clinical Case Manager to support our clients in New Bedford. The Opportunity: As a Case Manager, you will provide nonclinical support, educating clients and caregivers in areas including health maintenance and preventative measures, assisting in healthcare management navigation, and accessing community support. You will advocate for your clients and facilitate the success of clients remaining safely in their homes. You will collaborate with internal field staff and nursing teams to provide exceptional care for diverse demographics. This role is a full-time role at 40 hours per week. Overtime pay is available after 40 hours. This role will be supporting clients of two New Bedford Housing residential buildings. No client travel needed.

Requirements

  • Willingness to travel within assigned region required.
  • Valid driver’s license and reliable transportation required.
  • Bachelor’s degree in human services, in public health or another related field required.
  • A minimum of two years’ recent experience in the direct care of elders or adults with disabilities required.
  • Knowledge of aging process, elder/disabled delivery systems and an understanding of disability issues.
  • Ability to work independently, ethically and maintain professional boundaries.
  • Ability to interview, assess effectively and provide crisis intervention.
  • Ability to interact with and access in-house and on-site services for residents.
  • Ability to work cooperatively with community partners and stakeholders.
  • Strong communication, documentation and organizational skills required.

  • CPR Certification.

Nice To Haves

  • Fluency in both English and second language strongly preferred (Spanish, Cape Verde Creole, Portuguese)
  • Case management, behavioral health, social services and/or community health worker experience preferred.

Responsibilities

  • Manage a diverse caseload of patients and clients with various healthcare needs.
  • Collaborate with internal field staff and nursing team to provide strong healthcare support and resources.
  • Assist with obtaining information and accessing other healthcare and community services.
  • Participate in multidisciplinary care plan creation and review and discharge planning.
  • Address concerns regarding environment and any changes in psychosocial status.
  • Participate in multidisciplinary team meetings and work collaboratively to address client needs.
  • Complete all required documentation per regulation and communicate. professionally with client, PCP and other healthcare providers when necessary.
  • Perform other duties within the scope of practice as assigned by management.
  • Have a consistent schedule and be onsite at the designated NBHA locations.
  • Complete, at minimum, annual outreach visits with each resident at the designated site.
  • Meet new residents within their first month of tenancy.
  • Complete new resident intake to assess the possible need for services and interests.
  • Work with NBHA property management staff to provide support to residents identified to be at risk and in need of services.
  • Conduct regular meetings with residents to obtain their input for programming and services.
  • Coordinate with community partners to link residents with community programs and services and to provide enrichment programs/activities for residents.
  • Ensure that residents are informed of onsite programs and activities via the dissemination of flyers, literature, phone calls or other forms of outreach.
  • Complete and submit all documentation and reporting metrics as required by EOHLC/NBHA.
  • Participate in NBHA Resident Services group meetings.
  • Maintain the facilities in neat, and orderly manner. No alterations to space can be made without the direct consent of the NBHA administration.
  • Attend bi-weekly meetings with the property management and resident services.
