About The Position

The Case Manager assesses, plans, implements, and evaluates patients’ needs to help ensure they receive appropriate services and support. The role collaborates with stakeholders to facilitate effective discharge planning and improve client outcomes. The position is scheduled Monday through Friday, 8:00 a.m. to 4:30 p.m. An active RN license in the State of Illinois is required.

Requirements

• Associate Degree in Nursing (ADN).
• Active Registered Nurse (RN) license in the State of Illinois.
• BLS or CPR certification within 90 days of hire.

Responsibilities

• Advocate for patients’ needs and rights within healthcare and social services systems.
• Conduct comprehensive assessments to determine patient needs, healthcare goals, and eligibility for services.
• Coordinate services and resources, including referrals to healthcare providers, community resources, and support services.
• Deliver termination-of-benefits letters as determined by third-party payers, ensuring patients understand their rights and obligations; ensure family members or significant others understand when a patient cannot understand or comprehend.
• Develop individualized care plans in collaboration with patients and relevant professionals.
• Maintain accurate, up-to-date records of patient interactions, assessments, and service plans.
• Provide patients and families with information about services, resources, and strategies to manage their conditions.
• Monitor patient progress regularly and adjust discharge plans as necessary.
• Work with multidisciplinary teams to ensure comprehensive care and support.
