About The Position

Avenues Recovery Center is hiring a Case Manager for its inpatient detox and residential treatment facility in Fort Collins, Colorado. The Case Manager initiates and provides ongoing case management for clients throughout treatment, advocates for clients and addresses barriers to care, oversees insurance coverage, and coordinates resources and discharge plans to support clients after treatment.

Requirements

• Warm, empathetic, and respectful personality.

Nice To Haves

• High school diploma or GED preferred.
• At least one year of experience in the treatment industry preferred.
• If in recovery, at least one year of sustained sobriety is required.

Responsibilities

• Advocate for clients by addressing and resolving external barriers to treatment.
• Initiate case management for each client upon intake and address pressing needs.
• Oversee client accounts and ensure insurance remains active throughout treatment.
• Provide ongoing case management throughout the duration of treatment.
• Arrange post-discharge resources as necessary, including housing, financial aid, legal aid, and vocational training.
• Facilitate comprehensive discharge planning, including continuing care, housing, and sober support.
