About The Position

The Case Manager works within Homestretch, an independent housing program located in Ventnor, New Jersey. The role supports consumers by assessing their needs, developing and implementing individualized service plans, providing life-skills instruction, and helping consumers achieve transition goals. The Case Manager also provides crisis and behavioral interventions when needed, coordinates with service providers, maintains electronic health care records, submits reports, attends meetings and supervision, and maintains required service levels.

Requirements

• Bachelor’s degree from an accredited institution in Social Work, Psychology, Counseling, or a related field, with clinical training.
• Valid driver’s license in good standing.
• Successful completion of pre-employment drug screening.

• Experience with youth living in foster care is preferred.

Responsibilities

• Engage consumers and assess their needs.
• Develop individual service plans based on multiple assessments, addressing housing, education, employment, health, and permanency.
• Provide life-skills instruction through multiple methods of instruction.
• Assist consumers in accomplishing transition-plan goals.
• Provide crisis intervention when needed.
• Provide behavioral interventions when needed.
• Maintain collateral contacts with other involved service providers.
• Assist consumers in making linkages to needed services.
• Maintain electronic health care records.
• Submit monthly, quarterly, and semi-annual reports.
• Attend supervision as needed.
• Attend intra-agency and interagency meetings as required.
• Maintain required levels of service.
