About The Position

Community Care is hiring a full-time Case Manager to support elderly individuals and adults with physical and intellectual disabilities in living as independently as possible in the community. The role manages an assigned caseload in collaboration with an RN Case Manager and provides member-centered assessment, care planning, coordination, and service monitoring. The position is located in Washington County and also serves Washington and Dodge Counties, with regular local travel required. The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m.

Requirements

• Bachelor’s degree in Human Services or a closely related area; alternatively, at least three years of experience working with seniors or adults with physical and/or intellectual disabilities may substitute for a non-human-services degree.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel, requiring use of a personal vehicle, a valid driver’s license, and proof of required insurance coverage.

Nice To Haves

• One year of experience with older adults or people with physical and/or intellectual disabilities in a social service capacity is preferred.

Responsibilities

• Manage an assigned caseload of program members in collaboration with an RN Case Manager.
• Complete comprehensive member assessments, member-centered care plans, and functional screens.
• Provide ongoing care coordination and quality assurance monitoring of services.
• Maintain required documentation in accordance with established standards.
