About The Position

At Meridian Services, Case Managers advocate for clients and help them access resources and services that meet their individual needs. The role supports diverse populations, including people with intellectual disabilities, traumatic brain injuries, CADI and mental health diagnoses, and elderly or alternative care needs. Positions are based in Golden Valley or Hopkins, Minnesota. Employees work typical daytime business hours, Monday through Friday, and may work from home up to two days per week after six months of employment. Select remote opportunities are available in Stearns, Itasca, and Chisago counties; candidates must be located in Minnesota. Starting salary is $51,000 annually, with a higher salary available for candidates with a Bachelor’s or Master’s degree in Social Work. The stated salary includes full-time benefits, PTO, and a hybrid schedule.

Requirements

Be a licensed Social Worker, requiring a Bachelor of Social Work for licensure; or be a licensed Graduate Social Worker, requiring a Master’s in Social Work for licensure, as stated by the Minnesota Board of Social Work; or have a four-year degree in Human Services, Psychology, Sociology, or a related field.
Successfully clear the Department of Human Services background check.
Successfully clear the Motor Vehicle Background Check and have an acceptable driving record per company policy.
For remote opportunities, be located in Minnesota.

Responsibilities

Visit with people on the caseload and attend team meetings.
Complete or attend needs-assessment meetings and individual service plan development (CSSP).
Assist with planning for new service development.
Locate residential, vocational, and other needed services based on each person’s needs.
Monitor service delivery and ensure each person’s health and safety needs are met.
Complete referrals and provide crisis intervention as needed.
Develop waiver budgets.
Complete case notes.
Act as an advocate for people receiving services.
